The Annapurna region has real trekking risks. Annapurna Base Camp is not the same as climbing Annapurna I. ABC is a non-technical trek to 4,130 m. Its main risks include altitude sickness, cold, snow, rain, and falls. Slippery stone steps can also cause injury.
Higher parts of the Sanctuary can face avalanche danger in bad conditions. Nepal Tourism Board warns trekkers about illness above 3,000 m. The Himalayan Rescue Association also stresses slow ascent and early descent when symptoms get worse.
Annapurna I is a very different activity. It is an 8,091 m expedition mountain with a severe death record. The Himalayan Database lists 75 deaths on that peak. Do not use that climbing number to describe the ABC trek.
For trekkers, good weather choices, steady pacing, and a safe route plan reduce risk greatly.
